The first assessment

Shoulder pain after lifting may involve the rotator cuff, bursa, joint, muscle or another structure. A clear diagnosis cannot be made from the mechanism alone. Assessment considers pain location, movement, strength, work demands, trauma and whether medical review or imaging is indicated.

Early priorities

  • Modify painful lifting or overhead work
  • Maintain comfortable shoulder movement
  • Begin tolerable muscle loading
  • Address sleep and daily-task aggravation
  • Follow medical restrictions after major trauma or surgery

Progressive loading

Rehabilitation usually progresses from comfortable movement and low-load strength to heavier resistance and functional tasks. The program may include reaching, carrying, pushing, pulling and overhead work according to the role.

Pain response, strength, movement quality and tolerance across repeated tasks help guide progression.

Preparing for manual duties

A return-to-work plan should reflect load, height, frequency, pace and duration—not just whether a person can lift once. Temporary changes to weight, reach height or repetition may allow capacity to build gradually.

When further review is important

Arrange prompt medical assessment after a major fall or impact, visible deformity, sudden marked weakness, inability to raise the arm, unexplained swelling, fever or severe unrelenting pain.
