Neck pain and headaches: when are they connected?

Some headaches are influenced by the neck, while others are not. Assessment helps identify the pattern and the appropriate next step.

This guide provides general education and does not diagnose a condition or replace individual medical or physiotherapy assessment.

01

Can the neck cause a headache?

Pain from joints and muscles in the upper neck can be felt in the head. A neck-related headache may be associated with restricted neck movement, a consistent side of pain or symptoms aggravated by sustained posture. Headaches can have many other causes, so this pattern should be assessed rather than assumed.

02

What physiotherapy assesses

  • How the headache and neck pain began
  • Neck movement and symptom response
  • Strength and endurance around the neck and shoulder girdle
  • Workstation, driving, sleep and activity demands
  • Neurological symptoms or other warning signs
03

Treatment is more than posture correction

There is no single perfect posture that prevents all neck pain. Management may include comfortable movement, manual therapy, graded exercise and practical changes that reduce long periods in one position. Building tolerance is usually more sustainable than trying to hold a rigid posture all day.

04

Returning to work and training

Rehabilitation can progress from gentle movement to strengthening and the tasks that repeatedly provoke symptoms. Computer work, lifting, driving and gym exercises can be modified temporarily and then rebuilt.

05

Headache warning signs

Seek urgent medical attention for a sudden severe headache unlike previous headaches, headache after major trauma, new neurological symptoms, fever with neck stiffness, fainting, confusion, or headache with significant visual or speech changes.
